Just What Is Plaque?

From the time you were small, you’ve been warned about the dangers of plaque. Why? Because:

  • It’s an unappealing film that sticks to your teeth
  • It causes cavities
  • It causes gum disease

And really, do we need to know much more than this to motivate us to brush? But if you’re in a curious mood, you might be wondering just how this soft, fuzzy film accomplishes all that damage. Let’s take a closer look at the sticky problem of plaque.

How does plaque form?

We live with hundreds of species of oral bacteria, most of which are harmless, and some of which are actually beneficial. But when our oral ecosystem gets out of balance, problems can occur. For example, without regular and thorough brushing and flossing, we start to build up plaque.

Plaque starts forming within hours of your last brushing. And even though plaque fits the very definition of “seems to appear overnight,” this biofilm is actually a complex microbial community with several different stages of development.

  • It starts with saliva.

Saliva is vital to our oral health, because it keeps us hydrated, washes away food particles, neutralizes acids in the mouth, and provides minerals which keep our enamel strong. Saliva also contains proteins, which help form a healthy, protective film on the tooth surface. This film is called a pellicle.

  • Bacteria attach to the pellicle.

There are species of oral bacteria that are able to attach themselves to the pellicle film within hours of its formation. As they become more firmly attached, they begin to grow and divide to form colonies, and are known as the early colonizers of the plaque biofilm.

  • A complex biofilm forms.

If you’ve skipped brushing for a few days (please don’t!), you’ll notice a fuzzy, sometimes discolored film on your enamel—that’s a thriving plaque community, and it only takes a matter of days to go from invisible to unpleasant.

If you’re not removing plaque regularly, it can harden further and become tartar. And once you have tartar buildup, you’ll need the care of a dental professional to remove it.

  • What happens if we ignore plaque and tartar?

We get cavities and gum disease.

How does plaque cause cavities?

  • The bacteria in plaque, like all organisms, need nutrients.

Our normal oral environment and the food in our everyday diets provide the nutrients plaque needs. And, as we mentioned above, certain types of oral bacteria convert these nutrients into acids. Foods such as carbohydrates, starches, and sugars are most easily converted into acids, which is why we recommend that you enjoy them in moderation.

  • The biofilm promotes acid production.

Within the plaque film, anaerobic bacteria (bacteria which don’t use oxygen) convert sugars and starches into acids. As the plaque film becomes denser, it blocks acid-neutralizing saliva and oxygen from reaching these bacteria close to the tooth’s surface, creating an ideal environment for the bacteria to produce their acid waste products.

  • Acids attack enamel.

The sticky nature of plaque keeps these acids in contact with tooth enamel, where, over time, acids dissolve minerals in enamel, weakening the mineral structure of the tooth.

How does plaque cause gum disease?

  • Bacteria cause inflammation and gingivitis.

The bacteria in plaque irritate the delicate tissue of the gums, which causes an inflammation response which can leave your gums swollen, red, bleeding, or tender. This early form of gum disease is gingivitis. Fortunately, good dental care and careful brushing and flossing can usually prevent and even eliminate gingivitis.

  • Plaque and tartar can lead to periodontitis.

When plaque and tartar build up around and below the gumline, the gums pull away from the teeth, leaving pockets where bacteria collect, leading to infection as well as inflammation. Infections and constant inflammation not only harm gum tissue, they can destroy the bone supporting the teeth. This serious gum condition is periodontitis, and should be treated immediately to avoid further infection and even tooth loss.

How do we fight plaque?

From the time you were small, you’ve learned how to fight plaque:

  • Brush at least twice a day for two minutes, and be sure to brush all of your tooth surfaces and around the gumline.
  • Floss to remove plaque from between the teeth and near the gumline.
  • Visit our Granite Bay office for a thorough professional cleaning.

Happily, even with all there is to learn about plaque’s growth and development, it’s reassuring to know that getting rid of it is quite simple—with just a soft-bristled brush, some dental floss, and a few minutes of your time each day, you’re on the way to a healthy, happy, plaque-free smile.

Brushing Your Toddler’s Teeth

At Jerry Martin, DDS, we know that brushing your toddler’s teeth can be an intimidating prospect. So we’re providing a few tips in the hope of making the process a lot more easy, effective, and all-around enjoyable for everyone!

Start by getting into a position that gives you control and enables you to see well into your child’s mouth. If you can see clearly, you will be able to maneuver the toothbrush better around your son or daughter’s mouth for a better quality of brushing.

It’s important to choose a time when your toddler is calm. Have your little one sit with his or her favorite stuffed animal, or play a fun movie in the background so your child can focus on something comforting while you’re brushing.

Using a circular motion, brush all sides of their teeth. Be sure to let your toddler have a turn after you’re done, to start getting used to it. This way, he or she is more likely to repeat the brushing and flossing exercise when your youngster is old enough.

Dr. Jerry Martin and our team agree that brushing and flossing need to be performed with kindness and care. To ensure your child learns good dental hygiene habits early on, be gentle and make this time a happy, learning time.

What to Do When the Tooth Fairy Isn’t on Schedule

August 22 is National Tooth Fairy Day! If the Tooth Fairy is a treasured part of your child’s life, you’re just in time to celebrate! But what to do when the Tooth Fairy doesn’t arrive on schedule—when baby teeth stay longer than expected or are lost too soon? 

A baby’s 20 baby teeth tend to come in within a fairly predictable time frame. The bottom central incisors in the very front of your baby’s mouth typically make their appearance first, when your baby is around six to ten months old. Over the next few years, the remaining incisors, canines, and first molars arrive. Last on the scene are the second molars, which usually show up between the ages of 23-33 months. 

Just as baby teeth follow a pattern coming in, they tend to follow the same pattern falling out. The front teeth begin to wiggle and loosen around age six or seven, while the last of the baby teeth, the canines and second molars, are often lost between the ages of ten and 12. 

Baby teeth fall out as the adult teeth below them push up as they erupt. The top of the new tooth puts pressure on the root of the baby tooth, gradually dissolving it. As the root grows smaller and can’t anchor the tooth, the tooth begins to wiggle and eventually becomes loose enough to fall out. This leaves the adult tooth perfectly placed to grow into its proper position.

Sometimes, though, teeth linger far past their fall-out date. Sometimes, because of decay or trauma, they are lost much too early. In either case, Dr. Jerry Martin can provide treatment to protect little smiles now and to ensure that there’s space for the permanent teeth to erupt and align correctly.

Teeth Which Overstay Their Welcome

If that baby tooth never gets wiggly, the team at Jerry Martin, DDS can help! When baby teeth stubbornly hang on, adult teeth can erupt behind them, creating a double row of teeth commonly known as “shark teeth.” These permanent teeth can become crowded or misaligned as they try to fit in any space available. Or a baby tooth can block an adult tooth from erupting at all. When that baby tooth just isn’t budging, an extraction will create space for the permanent tooth to erupt.

Extracting a baby tooth is generally a straightforward procedure because primary teeth have very small roots. Your child’s dental team at Jerry Martin, DDS are experts in helping you prepare your child for the procedure in a gentle, reassuring, and age-appropriate way. 

Often, a local anesthetic is all that’s necessary for a simple extraction, but if you feel sedation would better fit your child’s needs, discuss sedation options with your dentist. After the extraction, you’ll be given clear information on how to deal with pain and swelling, which foods and drinks are best while the extraction site heals, and how to protect the area.

Teeth Which Exit Too Early

In the case of decay or trauma, Dr. Jerry Martin will do everything possible to save the tooth. When decay is so extensive that there’s not enough structure left to hold a filling or crown, or when there’s an infection in or around the tooth, or when an accident or injury has caused serious damage, extraction might be the healthiest option. Depending on your child’s age, further treatment might be needed afterward to protect future smiles. 

Besides their roles in eating and speaking, baby teeth save space for permanent teeth. Whenever a child loses a primary tooth too soon, the family dentist will be on the lookout for potential future orthodontic problems. If remaining baby teeth shift, taking up part of the empty space left behind by the lost tooth, the adult teeth below won’t have the space they need to erupt properly. Permanent teeth might come in at an awkward angle or in the wrong place. 

In this case, Dr. Jerry Martin might recommend a space maintainer. Space maintainers are small, custom-designed appliances which prevent the remaining baby teeth from shifting position while holding space open for the permanent tooth to arrive right on schedule and right where it belongs. 

It’s comforting to have charts which let us know when little teeth will typically come in and when they will typically fall out. But unexpected events might mean teeth overstay their welcome or depart too soon. Oral Cancer Facts and Figures

Oral cancer is largely viewed as a disease that affects those over the age of 40, but it can affect all ages, even non-tobacco and alcohol users. Oral cancer can occur on the lips, gums, tongue, inside lining of the cheeks, roof of the mouth, and the floor of the mouth. Our team at Jerry Martin, DDS recently put together some facts and figures to illustrate the importance of visiting our Granite Bay office.

Our friends at the American Cancer Society recommend an oral cancer screening exam every three years for people over the age of 20 and annually for those over age 40. Because early detection can improve the chance of successful treatment, be sure to ask Dr. Jerry Martin and our team to conduct an oral exam during your next visit to our Granite Bay office.

  • Symptoms of oral cancer may include a sore in the throat or mouth that bleeds easily and does not heal, a red or white patch that persists, a lump or thickening, ear pain, a neck mass, or coughing up blood. Difficulties in chewing, swallowing, or moving the tongue or jaws are often late symptoms.
  • The primary risk factors for oral cancer in American men and women are tobacco (including smokeless tobacco) and alcohol use. Risk rises dramatically (30%) for people who both smoke and consume alcohol regularly.
  • Oral cancers are part of a group of cancers commonly referred to as head and neck cancers, and of all head and neck cancers they comprise about 85% of that category.
  • Oral cancer is the sixth most common cancer among men.
  • Oral cancer is more likely to affect people over 40 years of age, though an increasing number of young people are developing the condition.
  • Death rates have been decreasing over the past three decades; from 2004 to 2008, rates decreased by 1.2% per year in men and by 2.2% per year in women, according to the American Cancer Society.
  • About 75% to 80% of people with oral cavity and pharynx cancer consume alcohol.
  • The risk of developing oral cavity and pharynx cancers increases both with the amount as well as the length of time tobacco and alcohol products are used.
  • For all stages combined, about 84% of people with oral cancer survive one year after diagnosis. The five- and ten-year relative survival rates are 61% and 50%, respectively.
  • It is estimated that approximately $3.2 billion is spent in the United States annually on treatment of head and neck cancers.

Cancer can affect any part of the oral cavity, including the lip, tongue, mouth, and throat.
